Faqs: Expired H1N1 Vaccine Form

Many facilities have begun vaccinating their residents and employees with the H1N1 Vaccine. As you know facilities must keep track of all vaccinations and log them as per the NYSDOH. For some facilities, the vaccine will expire within the next few months. If so, then you must also document this as well. Attached is the necessary form.
